Appointments
You may schedule an appointment by calling our office at (817) 926-BONE (2663) or fax to: Burleson Location (817) 293-8860 or fax to: Fort Worth Location (817) 926-2351. Your appointment is valuable to both of us. If you are unable to keep you appointment, notify us as soon as possible. In the event that your physician is delayed by an emergency, we will be happy to reschedule your appointment at your convenience.Prescriptions & Renewals
If a prescription refill is needed please contact your pharmacy and ask them to fax a refill request form. All refill requests will be filled at the end of the day. If a refill is not available, please call our office at (817) 926-BONE (2663) during normal office hours. We are unable to refill prescriptions when the office is closed. Do not page doctors for refills. Thank You for your cooperation and understanding.Financial Policy
Lone Star Orthopaedic & Spine Specialists, LLP participates in most Health care plans. If we are participating providers for your insurance company, you will be responsible for your co-pay or deductible at the time of service and we will gladly file your claim for you.If we are not participating providers for your insurance company, you will be responsible for paying in full at the time of service and filing your own claim. It must be understood that the patient, not the insurance company, is ultimately responsible for the payment of bill/s. Those patients paying privately are responsible for services when they are rendered. If surgery becomes necessary you will need to contact our billing office. In the event that your account becomes past due we will make a concentrated effort to collect but when necessary we will employ the use of an outside collection agency. We accept cash, personal checks, Visa, Discover and MasterCard. There will be a $25.00 charge for all returned checks. If necessary returned checks are delivered to the districts attorneys’ office for collection.
It is very important that you inform us immediately of any changes in your health insurance coverage. Please bring your insurance card with you to each visit. If you are unable to present your card before seeing your doctor, the visit may be considered fee for service and full payment may be collected.
Preparing for Your Appointment
- If you are a new patient or an established patient with a new problem please arrive 30 minutes prior to your appointment time to fill out necessary forms.
- Bring your insurance card and drivers’ license.
- Bring any copies of medical records, x-rays and or MRI’s if you have had previous treatment for the injury for which you are being seen.
